Dr. Allen does not accept commercial insurance and is not an in-network provider for any insurance company. We do accept Medicare with a standard Supplement (not HMO) for Seniors that are accepted into the practice

Payment by cash, check, credit card, or Venmo is due at each appointment unless other arrangements have been made in advance.

Appointments and initial consultations cancelled or rescheduled at least 48 hours before your appointment time will not be charged. Appointments and initial consultations cancelled or rescheduled within 48 hours before your appointment time will be charged a 50% fee. Missed appointments will be charged 100% of the appointment fee.
